I’m Prabha Milstein, a Licensed Marriage & Family Therapist based in San Francisco’s Noe Valley. I help people quiet the noise of anxiety by healing its roots, through a compassionate, attachment-based approach grounded in EMDR therapy.

Why I Work with Anxiety

This work is deeply personal to me.

So many of my clients come to me saying, “I can’t stop overthinking,” or “I just want to feel normal again.” But anxiety isn’t a flaw or a failure — it’s a message from the body asking to be understood.

My role is to help you listen to that message with compassion, not judgment. Together, we’ll uncover the experiences – past or present – that keep your system in survival mode, so you can begin to feel calm, confident, and in control again.

How Anxiety Shows Up

Anxiety takes many forms. You might recognize yourself in one or more of these:

  • Constant overthinking or inability to “shut off” your mind
  • Fear of rejection or people-pleasing
  • Perfectionism and self-criticism
  • Physical tension, restlessness, or racing heart
  • Trouble sleeping or relaxing even when things are “fine”
  • Feeling detached or overwhelmed by emotions

Whatever shape it takes, anxiety is your body’s way of saying it’s been carrying too much for too long.

My Approach to Anxiety

Anxiety can’t be forced to disappear. It softens when you feel safe enough to let your guard down. Here’s how we work together:

Safety First

When clients come in anxious, I don’t rush to fix it. I want them to feel they can trust me, that this is a space where nothing bad will happen.

Mind-Body Connection

Anxiety lives in the body. Through grounding and somatic awareness, you’ll learn to notice where it shows up (such as a tight chest, racing heart, or shallow breath) and how to regulate those signals.

Attachment Repair

Many people with anxiety grew up feeling they couldn’t rely on others. Part of our work is learning that it’s okay to need, and to be met.

Integration & Ongoing Growth

Over time, you’ll notice your body feels calmer and your thoughts less reactive. “You start to respond to life, not react to it.”

What to Expect

Initial Session
We start by exploring what’s happening in your life right now – your symptoms, stressors, and goals for therapy.

Developing Safety & Regulation
Before any trauma processing, we build tools for grounding, self-soothing, and emotional regulation.

EMDR & Reprocessing
Once you feel ready, we use EMDR to gently access the memories or experiences connected to your anxiety.

Integration & Ongoing Growth
Over time, you’ll notice your body feels calmer and your thoughts less reactive. “You start to respond to life, not react to it.”
